Preparing for Your Appointment

Appointment booking: Setting up your initial appointment is made easy via our booking site, which is accessible through this website by simply clicking on a “get started” link or a “book an appointment” button. Once you arrive at the site, based on availability, choose your preferred day and time.

If you have any trouble with booking your appointment, you may reach out to the practice via email or telephone for assistance. While we attempt to address requests as quickly as possible, please expect up to a 48 hour turn-around-time on responses.

Once an appointment slot is requested, you will be met with a quick prescreening form that must be completed before your booking process is completed. Please complete this screening immediately - it will take just a minute or two.

Intake paperwork: Following your appointment booking request, and the practice’s approval of said appointment, you will receive a packet of intake forms to your email that MUST be completed no later than 48 hours before your appointment. The forms must be completed electronically and submitted back to the practice. Unfortunately, appointments are automatically cancelled if this paperwork is not completed and submitted by the stated timeframe - as the provider cannot legally hold the appointment without these provided documents. These forms also assist the provider in adequately preparing for your visit.

Appointment day: Once we receive the required documents, you will be notified via email with your appointment confirmation. Two days before your appointment, you will receive a reminder via text and/or email. The day of your appointment you will receive a 2nd reminder one hour prior to your appointment. For virtual visits you will also receive a Zoom link to the provider’s virtual office with that 1-hour notice.

For in-person visits, please aim to arrive 15 minutes early. After entering the main building, take the elevator to the 3rd floor. Upon exiting the elevator, you’ll see a sign that says “HQ.” Turn left and immediately take the first right into the alcove entrance. Continue down the hall to the waiting area, where you will find the reception desk. Have a seat, and your provider will meet you shortly. ***In-person visits are NOT currently available and are by appointment only. This is a shared business space and is not privately owned nor leased by the practice. Please do not arrive to this location without confirmation from the practice to do so.***

Attendance Policies

Life is unpredictable, and we understand that unexpected situations can interfere with your plans. While we do make exceptions in certain cases, please keep in mind that both your time and ours are valuable.

Every appointment slot represents an opportunity for someone in need of care. Missed or late-canceled appointments can prevent others from receiving the support they need.

To help maintain consistency and fairness, we have policies in place to support appointment compliance. If these policies are not followed, applicable fees will be assessed as outlined.

No-shows/Cancellations/Reschedules: To help ensure fairness and availability, we ask that all appointment cancellations/reschedules be submitted at least 24 hours in advance. Cancellations/reschedules made with less notice as well as no-shows, will likely result in a fee, as outlined in our policy.

If you are an established client who needs a prescription refill following a canceled appointment, we ask that you promptly reschedule for the next available time. Once your new appointment is confirmed by your provider and a prescription request is submitted, a bridge prescription may be provided to cover your needs until your rescheduled visit.

Lateness: We kindly ask that you arrive on time for your appointment—whether online or in person. Appointment times are limited, and we want to make sure we can use every minute to give you the care you need. While we will do our best to accommodate you, arriving 10 minutes or more late to a follow-up appointment, or 15 minutes or more late to an initial visit, may result in the appointment being canceled and a fee being assessed, as outlined in our policy.

Repeated violations of our attendance policies may prompt a re-evaluation of your current treatment needs, goals, and readiness for care. In some cases, continued non-compliance may lead to the discontinuation of services.

Controlled Substance Policy

Your safety and care are our top priorities!

Controlled substances are regulated by the Drug Enforcement Administration (DEA), and our prescribing practices are closely monitored. State laws also require additional steps when prescribing these medications. We want our clients to understand that we take this responsibility seriously.

Many medications used to treat conditions like ADHD, anxiety, and insomnia fall under the category of controlled substances. Because of the potential risks associated with these medications, they are prescribed cautiously and monitored closely to ensure your safety.

Certain medications—such as benzodiazepines (e.g., Xanax, Ativan, Valium, Klonopin) and non-benzodiazepine sleep aids or "Z-drugs" (e.g., Ambien, Lunesta, Sonata)—are prescribed only in specific cases. These are typically used for short-term stabilization while we work toward a sustainable, long-term treatment plan.

If you are currently taking controlled substances prescribed by another provider, please know that we will not automatically continue or duplicate those prescriptions. Our providers will conduct their own clinical assessment and make treatment decisions based on professional judgment. We are committed to addressing your needs while upholding safe, ethical, and evidence-based prescribing practices.
